Registered Nurse - RN Job Description SunBridge Healthcare is currently seeking a Clinical Case Manager/MDS Coordinator to join our team in Carmichael, CA. The MDS Coordinator is responsible for completion and transmission of the resident assessment; reviews and manages reimbursement; responsible to case manage residents covered by both Medicare and Managed Care insurance programs. Our MDS Coordinators, RNs, are specialists in their field and bring key skills to our centers and residents every day. Participate in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs) ? Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes; as required by Federal and State regulations ? Ensure completion and submission of MDS data within regulatory timeframes ? Coordinate the development of comprehensive assessments and care plans of each resident ? Conduct/coordinate facility daily PPS meetings and IDT Process and Claim review ? Assist with education and overall plan of care ? Cooperate with monitoring and audit functions and investigations Job Requirements ? Registered Nurse, RN, Licensed in the state of CA ? Minimum 1 year of MDS experience (residential long term care) ?
